OHIO DRUG & ALCOHOL TREATMENT PROGRAM

No matter the length of time, the combination of substances, or any other factors, anyone living with addiction can begin again and achieve a better life for themselves and their families.

Soon after admission, each patient connects with a treatment team who collaboratively work with each patient to develop a strong, customized recovery plan. We offer a number of options for substance use treatment. These options include therapy, either alone with the therapist or in a group, as well as fitness and wellness groups, education lectures and family engagement.

CONTINUITY OF CARE

The treatment plan places patients in the least intensive/restrictive treatment environment, appropriate to their therapeutic needs. Variable lengths of stay/duration of treatment are provided within a variety of treatment settings. A patient can expect to find the following five levels of care:

(1). Medically Managed Detoxification
(2). Residential/Inpatient Care
(3). Intensive Outpatient Treatment
(4). Outpatient Treatment
(5). Aftercare and Relapse Prevention

The Palms 24-hour treatment setting provides a continuum of services capable of meeting the needs of individual under various levels of care. We transition the levels of care throughout treatment in response to changing clinical needs and treatment goals.

As individuals move throughout the continuum in their recovery they transition to levels of care of greater or lesser intensity, depending on the patients clinical needs and treatment goals and based on the current DSM criteria and ASAM Patient Placement Criteria. An individual may begin with inpatient treatment only to find that a less intensive level of care, such as intensive outpatient treatment, is more appropriate. Decisions as to which services are provided and how intense they are will be based on the patient’s needs and desires.

DETOXIFICATION

Immediately upon admission, each patient is assessed to determine the level of detoxification services required. When medically indicated, patient detoxification will be managed on an outpatient basis prior to inpatient/residential treatment.

Patients assessed as requiring medically managed detoxification (inpatient) will be entered into an appropriate medical facility. The Palms has an association with Highland Springs in Cleveland, and University Hospital in Ravena. All patients must have 72 hours of monitored abstinence (inpatient or outpatient) prior to departure from detoxification.

RESIDENTIAL/INPATIENT CARE

The Palms offers long term inpatient treatment from drug addiction. After being assessed, a person diagnosed with substance abuse problems, is provided care according to ASAM Patient Placement Criteria with the least restrictive treatment plan possible, depending on specific needs. Duration of inpatient rehabilitation is customized to meet the specific needs of the patient.

The Palms residential inpatient program is designed to improve the patient’s ability to structure and organize the tasks of daily living, stabilize the individual’s substance use disorder symptoms, to develop and apply sufficient recovery skills, and to develop and practice prosocial behaviors such that imminent return to substance use upon transfer to a less intensive level is avoided.

The Palms residential inpatient treatment program is based on evidenced based therapies such as motivation enhancement therapy, 12-step therapist led counseling, cognitive behavioral therapy, and family/significant other involvement. The Palms residential inpatient treatment program has variable lengths of stay/duration based on the patient’s needs and desires.

The Palms residential inpatient treatment program includes a range of cognitive, behavioral and other therapies administered on an individual and group basis; medication management and medication education; counseling and clinical monitoring; random drug screening; planned clinical activities and professional services to develop and apply recovery skills; family therapy; educational groups; occupational and recreational therapies; art, music or movement therapies; physical therapy; and related services. Comprehensive, multifaceted treatment is also provided to individuals with psychological problems.

The Palms residential inpatient services for co-occurring disorders in a 24-hour treatment setting is appropriate for individuals whose primary problems involve emotional, behavioral, cognitive, readiness to change, relapse, or recovery environment concerns. Intoxication, withdrawal, and biomedical concerns, if present, are safely manageable in a 24-hour treatment setting. The California Palms’ residential inpatient care in a 24-hour treatment setting meets the Clinically Managed High-Intensity Residential Services level of care found in ASAM Level 3.5.

INTENSIVE OUTPATIENT TREATMENT

Upon completion of inpatient/residential the patient will be assessed to determine the level of continuing care, either outpatient treatment or a period of aftercare. The decision regarding continuing care whether outpatient or aftercare will be based on a current assessment of status and include establishment of a continuing care treatment plan identifying specific goals, interventions, and means to assess interventions.

The Palms intensive outpatient program is an evidenced based model incorporating motivation enhancement therapy, cognitive behavioral therapy, 12-step therapy and family therapy. The therapy is administered either alone with the therapist or in a group basis. The therapy includes fitness and wellness groups, education lectures and family engagement.

The Palms intensive outpatient substance abuse treatment program is designed for patients who do not require residential treatment but need more services and coaching than provided in traditional outpatient treatment, or for patients who completed inpatient treatment and need more continued care. The Palms intensive outpatient program is a 12 week program but an extended outpatient program is also available based on the patient’s needs and desires.

OUTPATIENT TREATMENT

The Palms Outpatient Treatment provides a structured outpatient treatment program for patients that can function in the community while receiving outpatient care. Decisions regarding aftercare are based on a continuing care treatment plan identifying specific goals and interventions.

The Palms outpatient treatment program has variable lengths of stay/duration based on the patient’s needs and desires. There is no one-size-fits-all outpatient program. We tailor the outpatient program to meet the patients individual needs.

The outpatient program is an evidenced based model of group sessions incorporating motivation enhancement therapy, cognitive behavioral therapy, 12-step therapy, wellness groups, education lectures and family engagement.

AFTERCARE AND RELAPSE PREVENTION

Upon completion of inpatient/residential or intensive outpatient the patient will be assessed to determine the level of continuing care required and begin a period of aftercare. Decisions regarding aftercare are based on a continuing care treatment plan identifying specific goals, interventions, and means to assess interventions. At a minimum, a relapse prevention plan will be created and agreed upon prior to discharge.

The aftercare program will access the viability of 12-step programs, support groups, sober living or individual or group counseling. Upon discharge, patients are armed with the contacts and resources needed to continue along the road to recovery. Patients who located outside Northeast Ohio are encourage to affiliate with their local support groups to ensure continuity of treatment. There is no one-size-fits-all aftercare program. We tailor an aftercare program to meet the patients individual needs.

The patient’s progress will be monitored while in aftercare. The goal is to ensure that the patient does not need to be returned to treatment and a relapse prevention plan is implemented and proceeding smoothly.

The Palms clinical team will determine when the patient no longer requires aftercare based on ASAM industry guidelines found in Treatment Criteria for Addictive Substance-Related and Co-Occurring Condition.
